How to Use the Return Form

1. Download and print the Return Form:
   
   • Download the Return Form (PDF, EN)
   • Pobierz Formularz Zwrotu (PDF, PL)
2. Fill in the required details – including your name, email, order number, and reason for return. Select the applicable reason from the list.
3. Place the completed form inside the return package together with the items being returned.
4. Send the package to the following address (at your own cost unless otherwise specified):
   
   DOTPACK – Paprocki&Brzozowski
   ul. Wspólna 1, 42-125 Libidza
   Poland
5. Returned items must be:
   • Unused and in original condition
   • With all tags attached
   • Properly packed to avoid damage in transit

We will process your refund within 14 days of receiving the returned goods.
